Billing & Insurance

Stephanie Butler, LAMFT PLCC is a fee-for-service practice where payment is due in full at the time of your session. We do not directly bill insurance. We are not contracted with any insurance providers and do not directly bill insurance.

Stephanie is an “out-of-network providers." If you have out-of-network benefits, you may receive some reimbursement for our services if you submit claims directly with your insurance company. It is the client’s responsibility to communicate with their provider and confirm these benefits.

Stephanie can provide a superbill directly to you that contains all appropriate service codes and diagnostic codes. Superbills would need to be submitted directly to your insurance provider for reimbursement by you. 

Clients may also use their Health Savings Account cards, cash, or major credit cards. Credit Card payments will have an additional processing fee to cover CC services. Fee is currently $4.50 per transaction.

Please note that Medicare, Medicaid and AHCCCS do not offer out of network benefits.
